The same rule of thumb applies: eat right and exercise. The question that many ask is, "What is the proper diet for me, or my body type, and how much exercise is enough?" This varies for everyone. CORRECTION OF PROMINENT EARS - Otoplasty
Ear surgery, or otoplasty, is usually done to set prominent ears back closer to the head or
to reduce the size of large ears.

CANDIDATES FOR OTOPLASTY:

The operation is done on children between the ages of 4 and 14 or adults at any age. Ears are almost fully grown by age four, and the earlier the surgery, the less teasing and ridicule the child will have to endure. Ear surgery on adults is also possible, and there are generally no
additional risks associated with ear surgery on an older patient.

CONSULTATION & PREPARATION:

Parents should stay alert to their child’s feelings about protruding ears; don’t insist on the
surgery until your child wants the change. Children who feel uncomfortable about their ears
and want the surgery are generally more cooperative during the process and happier with
the outcome.We will also give you specific instructions on how to prepare for surgery.

RISKS & COMPLICATIONS:

When ear surgery is performed,complications are infrequent and usually minor.
Nevertheless, there are risks associated with surgery and specific complications associated
with this procedure.

A small percentage of patients may develop a blood clot on the ear. It may dissolve
naturally or can be drawn out with a needle. Occasionally, patients develop an infection in
the cartilage, which can cause scar tissue to form. Such infections are usually treated with
antibiotics; rarely, surgery may be required to drain the infected area. Recurrence of the
protrusion, may require second surgery.

HOSPITAL STAY/OUT-PATIENT:

Ear surgery is performed as an outpatient procedure.

ANESTHESIA:

If your child is young, we recommend general anesthesia, so the child will sleep through the
operation. For older children or adults, we prefer to use local anesthesia, combined with a
sedative.

OPERATING TIME:

1-2 hours.

THE SURGERY:

The technique will depend on the problem.
A small incision is taken on the back of the ear to expose the ear cartilage. The cartilage is
sculpted and bent back toward the head. Non-removable stitches are used to help maintain
the new shape. Occasionally, a larger piece of cartilage is removed to provide a more
natural-looking fold when the surgery is complete.
Another technique involves a similar incision in the back of the ear. Skin is removed and
stitches are used to fold the cartilage back on itself to reshape the ear without removing
cartilage.

In most cases, ear surgery will leave a faint scar in the back of the ear that will fade with
time. Even when only one ear appears to protrude, surgery is usually performed on both
ears for a better balance.

THE RECOVERY PERIOD:

Adults and children are usually up and around within a few hours of surgery.
The patient’s head will be wrapped in a bulky bandage immediately following surgery to
promote the best molding and healing. The ears may throb or ache a little for a few days,
but this can be relieved by medication.

Within a few days, the bulky bandages will be replaced by a lighter head dressing similar to
a headband. Be sure to follow our directions for wearing this dressing, especially at night.
Stitches are usually removed, or will dissolve, in about a week.
Most adults can go back to work about 5 days after surgery. Children can go back to
school after 7 days or so, if they’re careful about playground activity.

THE NEW LOOK:

Most patients, are thrilled with the results of ear surgery. But don’t expect both ears to
match perfectly-perfect symmetry is both unlikely and unnatural in ears. If you’ve
discussed the procedure and your expectations with us before the operation, chances are,
you’ll be quite pleased with the result.

OTHER EAR PROBLEMS

Besides protruding ears, there are a variety of other ear problems that can be helped with
surgery. These include: “lop ear,” when the tip seems to fold down and forward; “cupped
ear,” which is usually a very small ear; and “shell ear,” when the curve in the outer rim, as
well as the natural folds and creases, are missing. Surgery can also improve large or
stretched earlobes, or lobes with large creases and wrinkles. Surgeons can even build new
ears for those who were born without them (microtia) or who lost them through injury (ear
reconstruction)

Skin Rejuvenation with Chemical Peels





What is a chemical peel?

A chemical peel is a skin rejuvenation technique designed to improve the appearance of the skin. During a peel, a uniquely formulated solution is applied to the skin surface, causing the surface cells to separate and peel off, thus allowing new skin to regenerate. The new skin is generally smoother and less wrinkled.

Following a chemical peel, the skin color may also be more uniform.



What can a chemical peel accomplish?

Chemical peeling can be used to treat fine lines, especially around the eyes and mouth, and the wrinkles that result from sun damage. Chemical peeling can also be used to improve the skin’s color. Thus, sunspots, age spots, and the blotchiness that can occur with birth control pills or pregnancy (melasma) can all be improved by chemical peeling. In addition, the pre-cancerous skin changes that result from excessive sun exposure (actinic keratoses) can also be improved with a chemical peel. The procedure can also be used to improve acne and to treat mild scars that were caused by acne. A chemical peel will make dull appearing skin appear more vibrant and revitalized.



Who are the best candidates for a chemical peel?

The best candidates have fine lines and wrinkles with minimal skin sagging. They do not have severe skin excess. Patients with lighter complexions are preferred because there is less of a chance of the skin getting darker (known as hyperpigmentation) following the procedure.



What areas of the body can be treated with a chemical peel?

A chemical peel may be performed on the face, neck, chest, hands, arms, and legs.



Why are there so many different kinds of chemical peels?

Physicians have developed many different solutions for chemical peeling in order to provide patients with a choice of treatment options. With so many different peels available, the physician and patient can select a peel designed specifically for what the patient wishes to improve and the amount of downtime the patient can allow.



How do I decide which kind of chemical peel is best for me?

A peel should be selected based upon the type of skin condition that needs improvement. If the skin changes are confined to the upper layers of the skin, then a superficial peel will be effective. However, if the changes in the skin involve the deeper layers of the skin, then a deeper and more aggressive peel will be required to obtain improvement.



Which skin changes are superficial and which skin changes are deeper?

Changes limited to the superficial layers of the skin include increased skin pigmentation and a roughened skin texture without wrinkles. When the textural changes include wrinkling around the eyes and mouth, the changes in the skin have extended deeper into the skin. The most severe changes in the skin include generalized deep wrinkling with a leathery appearance and feel to the skin. These changes imply still deeper damage to the skin.





What are the different types of peels that are available?



Superficial peels are commonly performed with either TCA (10-25%) or Jessner’s solution. Salicylic acid can also be used for superficial skin peeling. It has been used for decades and is a good peeling agent for patients who also have acne. Alpha hydroxy acids, such as glycolic acid at 30-50%, are also commonly used for superficial peeling.



Medium depth peels are usually done with TCA (35-50%), glycolic acid (70%), or 35% TCA augmented with Jessner’s solution or with glycolic acid.



Deep chemical peels are generally done using phenol. Deep peels are only done in individuals who have significant changes in the skin, and have deep wrinkles. As a general rule, the deeper the peel, the longer the healing time. Thus, full face deep chemical peels will have the longest healing time, and the redness can last for as long as six months. In addition, deep chemical peels have a higher incidence of temporary and permanent pigmentation changes. For these reasons, relatively few deep chemical peels are performed today.

Due to the increased risk with deep chemical peels, many patients prefer to undergo repetitive superficial and medium depth peels. By repeating their peels, patients obtain additional improvement that is far beyond what only one lighter peel can accomplish.



How long is the recovery period following a chemical peel?

A superficial peel has a recovery period of a few hours to a few days, depending on how aggressive the treatment was. Recovery from a medium depth peel generally takes about one week. A deep chemical peel will take from several weeks to several months to completely recover.



Is a chemical peel a painful procedure?

Generally speaking, a light to medium depth chemical peel is not painful. Most patients experience a warm to somewhat hot sensation that lasts for five to ten minutes. This may be followed by a mild stinging sensation. However, a deep chemical peel may require pain medicine during and after the procedure.



When is chemical peeling not indicated?

Wrinkles that occur during muscle action (known medically as dynamic wrinkles), deep scars, and skin sagging due to aging are generally not improved with chemical peeling. For this reason, other procedures should be used to treat these problems

Suspecious Moles or Lesions on Your Body? What You Should Know...



Not unless your Cindy Crawford, notoriously well-known for her striking beauty and also for her distinguished mole, you probably shouldn't consider new or irregular moles to be a good thing.

First, let's explain the difference between moles and lesions. A mole is typically a small mark or spot on your skin. Perhaps this small mark is even raised. This may be any small pigmented patch, a freckle, or even a skin tag.
Lesions are typically larger (more than a centimeter) and are typically changing or growing over time. A skin a lesion, is typically more complex to manage than a mole, as may also be a skin cancer.

It's important to self-exam your own skin and to be familiar with any particular markings or moles you may have, and any new ones that may appear. Also, it's important to recognize if their is any change (especially increase) in size. Regular physical exams by your doctor is always the best recommendation.

Moles and lesions are treated under local injection anesthesia. Moles are generally treated with by shaving it. They often heal inconspicuously , with little evidence of scarring. A lesion is more likely to be excised, which may result in a small, linear, imperceptible scar. *These treatments may be covered by your insurance.

MACS:( Minimal Access Cranial Suspension) -Short Scar Face Lift






Men and women with signs of aging in the face such as jowls, deep marionette lines and sagging necks now have an alternative to a facelift: the MACS-lift. Developed in 2002, the MACS-lift, short for “Minimal Access Cranial Suspension,” involves lifting muscle and tissue vertically and fixing them in a more youthful position with dissolvable sutures before gently redraping the skin over them. Dr. Drew jumped in first, asking if Botox is dangerous. "Botox is safe. It is a neurotoxin but it's a purified protein," said Dr. Layke.











At what age should you go for plastic surgery? Dr. Danielpour answered, "The new trend is to avoid the wrinkles. Try to get in before the aging occurs, so you don't have to have the major facelifts when you get older. Your mid-20s is a good time to look for a little bit of facial rejuvenation."
Dr. Layke also said there's a trend -- for couples. "There are couples coming in to look younger together. It's no longer just the female coming in for a mommy makeover, now it's a little nip/tuck from the male's perspective. Couples are coming in for a facial rejuvenation."
Dr. Drew asked about the different procedures for breast implants. "There are three ways," Dr. Layke explained. "One's under the muscle and that's really done so you can mask the implant. The other way is to put it under the gland, under the breast tissue itself, that's for someone who really has breast tissue to begin with. It's very natural appearing and down time after surgery is very minimal. And there's a third way called a subfascial approach. It's really a combination of the two -- a connective tissue layer that's over the muscle but under the breast tissue."

Radiesse - Non-Surgical Options



Radiesse is one of the "filler" products that The Beverly Hills Plastic Surgery Group is offering to soften and reduce some of the facial lines such as the nasolabial folds (the lines between your nose and mouth) and the marionette lines (the lines that run down from your mouth). Radiesse can also be used to reshape the jaw line. Like other fillers, these results can be achieved with a simple in-office visit. The advantage of Radiesse over some of the other existing "fillers" is that it lasts on average for 1 year. Some of the other existing "filler" products only last on average six months (Restylane® and Perlane®) and some only three months (Collagen products).



The results with Radiesse can be seen immediately. As with any "filler" treatment or injectable there may be some bruising for a few days. Makeup usually adequately covers most bruising and patients are able to go about their normal daily routine. While this bruising may be minor, patients do need to be aware of it and not schedule treatments within a week of "important events."

Radiesse is a safe, biocompatible, biodegradable, injectable that is made up primarily of Calcium. The FDA approved Radiesse in December 2006 for the correction and filling for moderate to severe facial folds and wrinkles, including nasolabial folds. No skin testing is required.

Radiesse is gently injected into the skin in small amounts with a very fine needle. Its exclusive calcium-based microsphere technology contains tiny particles that form a scaffold around which the body is stimulated to produce new collagen. This unique response provides immediate, visible improvement of folds and wrinkles, while the body's own collagen naturally restores a fuller, more youthful appearance that keeps patients looking better even longer. Radiesse's tiny microspheres gradually break down into calcium and phosphate ions that are naturally taken up by the body over time.

Women or men who have facial wrinkles and folds common with aging and volume loss are candidates for Radiesse. In particular, Radiesse is the better choice for women who are familiar with other filler products but are frustrated by the inconvenience of injections every 3 to 6 months. In addition, these patients need to be willing to pay the slightly higher cost for the Radiesse injection. However, in the long run, Radiesse is more cost effective when you consider that it lasts on average twice the length of time as Restylane. In addition, there is slightly more material in the Radiesse syringe.
Radiesse is not recommended for use in enlarging or enhancing the lips. At BHPSG, Juvederm is used for lip augmentation.

Since Radiesse has no anesthetic in the product we recommend that patients have a "dental block" before they receive an injection of Radiesse. We use a technique that is similar to the technique that dentists use when giving "Novocain" for dental work. This "dental block" is identical to the "dental block" that is used before injecting other fillers. We provide this "dental block" at the time of treatment and it is included in the price of a Radiesse injection.

Beverly Hills Plastic Surgery Group offers Radiesse at a very attractive price of $700 for a 1.3 ml syringe. Two syringes of RadiesseTM 1.3 ml syringe injected at the same visit costs $1300 ($100 discount). Prior to receiving their first treatment of Radiesse a patient would need to have a consultation with Dr. Danielpour and Dr. Layke. Umbilical Hernia Repair



What is the cause of an Umbilical Hernia?


Umbilical Hernias can be acquired as the result of a repetitive or sudden injury, strain or pressure which causes the abdominal wall to weaken. However, umbilical hernias can also be since birth, otherwise known as congenital. In some cases, umbilical hernias are caused at birth by a weakness in the navel area where the umbilical cord left the infant and as a result, the umbilical ring never quite heals. What exactly prevents the umbilical ring from closing entirely is not known, though it is thought that many aspects affecting the developing fetus are involved. Umbilical hernias can be commonly found in infants, but can occur at any time in later adulthood.


As for children or infants, hernias may close slowly, possibly delaying surgery unless the hernia causes problems. Umbilical hernias which do not close or heal on their own may require surgery. In adults an umbilical hernia will not heal and go away and is often caused by sudden or repeated strain or stress on the abdominal muscles. As time passes, the hole or weakness increasingly grows larger allowing more abdominal contents to protrude, sometimes to the point that the hernia will distort the navel creating an “outie” instead of an “innie”. Umbilical hernias in adults can become problematic in that strangulation can occur and immediate medical attention may become necessary.


Signs and symptoms of an umbilical hernia vary for each individual, however, the primary symptom is often a small, soft bulge under or around the navel area. The bulge may be visible, or it may only be felt when pushed on. Sometimes the umbilical hernia is accompanied by pain, or a burning sensation, in the abdomen and may become more severe when lifting, coughing or sneezing. This area may also become swollen and may appear red or a gray-blue on the surface. It is possible to be able to push part of the bulge from the hernia back into the abdomen temporarily, but again, it will not heal or go away on its own and will eventually require surgery.

Gynecomastia: Enlarged Breasts in Men



Shame. Humiliation. Insecurity. These are some of the emotions experienced by men with enlarged breasts, or gynecomastia.



About half of all men today have excess localized fat and/or excess glandular tissue in one or both breasts. Gynecomastia accounts for more than 65 percent of all male breast disorders, according to the American Society of Plastic Surgeons (ASPS).




Causes of Enlarged Breasts in Men




In most cases, the cause is unknown. Sometimes it is an imbalance of the sex hormones estrogen and testosterone. But heredity conditions, diseases including certain endocrine disorders, tumors, and chronic diseases or certain drugs such as anti-depressants, blood-pressure medicines, marijuana, and steroids may also cause the condition.

Some men or boys have fat on their chests that makes it look as though they have breasts. This is not gynecomastia, but is called pseudogynecomastia and may go away with diet or exercise. Some men or boys will need treatment for this, as these fat pockets can be resistant to the effects of diet and exercise. In contrast, true gynecomastia will not usually improve with diet and exercise.

While not a serious medical problem, enlarged breasts can cause pain. They can also cause embarrassment — especially among young boys who may be teased as a result of their male breasts. It is especially common in teen boys. Up to 65 percent of 14-year-old boys have enlarged breasts, according to the American Academy of Family Physicians. This is largely due to the fluctuations in sex hormones that occur during puberty. The excess breast tissue disappears without treatment in two years in 75 percent of these boys, and within three years in 90 percent of them.

Repeated use of products containing lavender oil or tea tree oil may spur breast growth in prepubertal boys, according to a study in The New England Journal of Medicine. The effects fade when boys stop using the products.

More than half of male infants are born with enlarged breasts due to the effects of their mother's estrogen. This typically disappears within two to three weeks after birth.

In older men, gynecomastia often results from decreasing levels of the male sex hormone testosterone, which occurs with advancing age. In addition, men experience an increase in body fat with age and a resultant rise in the ratio of estrogen to the male sex hormones (androgens).
Gynecomastia Treatment

Surgery, liposuction or a combination of the two may be used to treat enlarged breasts. The reasons for surgery are two-fold in men: The first goal is to reconstruct the contour of the chest. The second goal is to determine if a suspicious lesion in the chest is cancerous. Enlargement typically occurs on both sides of the chest. If it is one-sided, firm and hard, see a doctor immediately to rule out male breast cancer.

The first step for treatment is a consultation with a fully-trained plastic surgeon. This evaluation will determine if you are a good candidate for male breast reduction.

You may be a candidate if you are healthy, are of relatively normal weight, have realistic expectations, are bothered by the feeling that your breasts are too large and (this is important) your breast development has stabilized. In general, the best candidates for surgery have firm, elastic skin that will reshape to the body's new contours.

Teens may benefit from surgery if their gynecomastia has been present for more than two years or if the problem is severe. In some teens, secondary procedures may be needed down the road if breast development continues.

Male breast reduction may be discouraged if you are obese, or if you are overweight and have not first attempted to lose the fat around the breast area with an exercise or weight loss program. If you use drugs that may cause the condition, you are not a good candidate. Your doctor would first advise you to stop using these drugs or to switch to other medications that do not cause it. Never stop taking a prescription medication without talking to your doctor first.
In addition to taking a thorough medical history, your surgeon will also assess whether you have any allergies or have had any prior surgeries in the chest region. He or she will also perform a physical exam to measure the extent of excess fat and glandular tissue in the chest area. Your surgeon will also likely conduct diagnostic testing to determine the underlying cause of your enlarged breasts. This will include tests of your endocrine function. The endocrine glands secrete the hormones that have been linked to enlarged breasts.

Your surgeon will likely take some "before" photos during this consultation process and will also discuss your expectations from the surgery.
